Insurance
I am an out-of-network provider for all insurance plans. I’m not paneled with any insurance companies; however, many people have out-of-network benefits and are pleasantly surprised to find out that they are eligible for either partial or full reimbursement for therapy. Coverage for my services will vary depending on your health insurance provider and specific plan.
To receive coverage through your out-of-network benefits, patients pay me in full and then submit a claim to their insurance. I will provide the necessary “superbill form” to submit for out-of-network claim filing. After the claim is submitted and filed, individuals receive a check for the reimbursement. This can often take 6-8 weeks, but it does work!
I recommend calling your insurance company and asking the following questions to determine the coverage that you have:
Does your insurance plan have out-of-network mental health (behavioral health) benefits?
Do you have an out-of-network deductible that has to be met first before you get reimbursed?
If you do have a deductible, has any of it been covered yet this year?
What is the "usual, customary, and reasonable fee" covered by your insurance for "outpatient psychotherapy (CPT code 90834)" in the zip code 10016?
How much of the fee does your insurance cover? This is usually a percentage
Is there a session limit?
Some insurance companies only cover a certain amount of sessions per year
What paperwork is required to submit claims for out-of-network reimbursement? (This is usually called a "superbill" and also a claims form, ask where you can find their claims form)
Where do you submit this paperwork? (sometimes it's online, sometimes it's by mail only)
Rates
During our phone consultation, we will discuss my fees and any specific needs that you have. If you have any concerns about fees or if my services are at a rate that you cannot afford, I will help you with referrals and connect you to alternate providers..
Payment
I accept cash, check, or Zelle. If you have a FSA/HSA card I can help you determine how to put that benefit towards sessions.
Policies
Confidentiality
Anything you share with me is fully confidential. You can provide me written consent if you’d like me to speak with another provider or family member. Legal exceptions to confidentiality are as follows:
In the case of suspected child or elder abuse.
In the case of a serious threat made to harm another individual.
In the case of a threat to cause oneself imminent harm.
Cancellation Policy
If you need to cancel or reschedule your appointment, please make sure to cancel at least 24 hours in advance. If you do not notify me at least 24 hours in advance and don’t attend your session, you are responsible for the session fee. I always try to find time to reschedule within the same business week in order to avoid charging for a missed session based on availability. Cancellations due to last minute emergencies or illness are handled on a case by case basis.
